Spanish language film (Latin American) starts with psychiatrist and patient in psychiatric hospital

Starts with scene in psychiatric hospital with dialogue between psychiatrist and patient,and psychiatrist thoughts are spoken over patient, very clever and amusing start to the film. I didn’t see much of the film and really want to know the title! It must have been made at least 10 years ago.
